Wegovy vs Rezdiffra for MASH: what is actually approved?
Both have U.S. FDA-approved uses in selected adults with MASH and moderate-to-advanced liver fibrosis. Rezdiffra is resmetirom, a thyroid hormone receptor-beta agonist; Wegovy injection contains the GLP-1 receptor agonist semaglutide. They are different medicines, and their separate trial results do not prove which is better for an individual.

|---|---|---|
| Active ingredient | Resmetirom | Semaglutide |
| Drug class | Thyroid hormone receptor-beta agonist | GLP-1 receptor agonist |
| Common misunderstanding | Not for every incidental fatty-liver finding | Not every semaglutide formulation carries the same MASH use |

Who is the treatment for?
The relevant approval is for a defined MASH population, not every person with fat seen on imaging. The prescribing information distinguishes fibrosis stage and cirrhosis status. Establishing whether someone fits that indication requires clinical evaluation. A person searching for “fatty liver medicine” should therefore check the diagnosis and stage before comparing brands. Having obesity, diabetes or an abnormal liver test may be relevant, but none automatically supplies all the information needed for this decision.
How the medicines differ
Resmetirom is an oral medicine aimed at a liver-related receptor pathway. Wegovy injection is a semaglutide product with metabolic and appetite-related effects and specific labeled uses. Route, coexisting conditions, interactions and monitoring can affect the choice. The Wegovy name also appears on different formulations and strengths: an approval for a particular injection does not mean every semaglutide preparation has the same MASH indication. Verify the current product label rather than extrapolating from the ingredient name.
What the approvals do not establish
Trials evaluated outcomes such as MASH resolution without worse scarring or fibrosis improvement without worse MASH. Those outcomes matter, but they are not the same as a demonstrated reduction in every future liver complication for every patient. Nor can percentages from separate trials be treated as a head-to-head contest: populations, trial designs and assessment times can differ. Claims that one medicine universally “reverses fatty liver better” go further than this comparison can support.
What makes this a clinical choice
The decision includes eligibility, potential adverse effects, other medications, pregnancy considerations and the monitoring plan. Lifestyle measures remain part of care rather than an alternative discarded once a prescription is started. Price and insurance access may matter, but a quoted cash price is not a reason to substitute a compounded or research-use-only product for the prescribed medicine. Discuss the specific product and indication with a qualified prescriber; do not use this page to switch or combine treatments.

Is Ozempic automatically approved for MASH because it contains semaglutide?
No. Approval is product- and indication-specific. The FDA MASH approval discussed here concerns Wegovy injection, not every product containing semaglutide.
Is either medicine a cure for all fatty liver?
No. These are treatments for particular patients. A reduction in liver fat, MASH resolution and improvement in fibrosis are different outcomes, and ongoing care remains important.
